Transplant peer support

Patient Support Offerings

Our team at UC Health is here to guide you through your transplant journey. Our multi-disciplinary team is here to

help answer any questions you may have from the beginning of your evaluation through post-transplant recovery.  We have many patient resources available.  They include:  Transplant peer support: Our patient-led team will pair you with another transplant recipient who shares similar circumstances. 

This is also available for caregivers who wish to connect with other caregivers.

SIGN UP

Monthly Transplant Support Group - These group provides an in-person opportunity for patients and their loved ones to connect and provide support for each other. 


UC Health Transplant Facebook Group – These groups provides in an informal virtual option to communicate with patients and caregivers. 

UC Health Liver Transplant  Group

UC Health Kidney Transplant  Group

UC Health Heart Transplant  Group

If interested in learning more about any of these options, please contact your transplant social worker for more information. If you do not have the contact information for your assigned social worker, please call the main transplant line and you will be connected.     


Remember, you are never alone in this journey and look forward to working with you and your caregivers.
